All applicants must submit their Intent to Apply proposals online. You will be asked to provide the following information:
- Organization name and address
- Web address and email
- Organization contact person and phone number
- Confirmation that the organization is a not for profit entity. You will need to provide the organization's Tax-ID number and upload documentation.
- Amount of request and time period
- Organization's total operating budget
- Program/Project total budget
- List of all committed and non-committed funding sources for the program
- Description of any previous Genentech funding (date and amount)
Please describe from the list below which Genentech Foundation focus area your organization or program fits into:
- Community
- Health Science Education (please specify if your focus is K - 12, higher education or both). If you are applying for fellowship funding, you will be asked to provide some addtional information
- Patients (please specify if your focus in on patient education or patient advocacy)
